Crystal Kelley Massage provides professional medical massage therapy services. All services are performed by licensed massage therapists in compliance with state and federal regulations.
2. Appointment Requests and Cancellations
Requesting Appointments: Clients may request appointments via phone, in person, or through our website. A member of our staff will follow up to confirm the appointment based on availability.
Cancellation Policy:
Cancellations must be made at least 24 hours in advance.
Late cancellations or no-shows may incur a cancellation fee of up to 50% of the service cost.
Exceptions may be considered for emergencies.
Late Arrivals: Arriving late may shorten the duration of your session, but you will still be responsible for the full service fee.
3. Client Conduct
To ensure a professional and respectful environment, clients are expected to:
Arrive clean and on time for appointments.
Communicate openly about any health concerns or preferences.
Treat staff and other clients with respect.
Refrain from inappropriate behavior, including harassment or solicitation.
Follow the therapist's instructions during sessions to ensure safety and effectiveness.
Violation of these guidelines may result in refusal of service or being banned from our practice.
4. Payment and Fees
Copay is due at the time of service when billing insurance.
Accepted Methods of CoPay Payment: Cash and all major credit cards.
Clients are responsible for any fees not covered by insurance.
5. Privacy and Data Protection
We are committed to protecting your personal information in accordance with our Privacy Policy and HIPAA regulations. Your data is used solely for the purpose of providing services and processing insurance claims.
6. Health and Safety Policies
Clients must disclose any medical conditions that may affect treatment.
We reserve the right to refuse or modify services based on health conditions.
If you are feeling unwell, please reschedule your appointment.
7. Changes to Terms
We reserve the right to update these Terms at any time. Changes will be effective upon posting on our website. Continued use of our services indicates your acceptance of the updated Terms.
